The Patient Health Questionnaire (PHQ-9)* is a brief measure of symptoms of depression, based on the diagnostic criteria for Major Depressive Disorder described in DSM-IV. The PHQ-9 consists of the following 9 questions. Please note: 1. These questions are designed to help you reflect on your situation or that of someone close to you. This should include providing contact details for emergency services (Police or Ambulance: ph 000), crisis services (Lifeline: ph 13 11 14), or the patient's General Practitioner if available.
